As they go into investigating the prison's paranormal past, guards and inmates alike have reported new paranormal activity. PRS brings in the medium Chip Coffey, as well as two demonologists, Lorraine Warren and Chris Moon, to help them investigate this prison.

There, Ryan and the group come into contact to what appears to be Mothman, the entity Ryan has been trying to investigate for his personal reasons. However, getting to the truth will not be easy, as the group has to go through burial grounds, government secret bunkers, and eyewitnesses.
